Resident Lease & Onboarding Packet

For approved residents only. Complete all sections below to finalize your lease and onboarding with RecoverPurpose.

01Resident Identity — Must Match Application

Use the same full name, phone, and email from your Housing Application so records can be matched.

03House Rules
1.Zero tolerance for alcohol, illegal drugs, or any mood-altering substances on the premises.
2.All residents must attend a minimum of 3 recovery meetings per week and provide verification.
3.Curfew is 10:00 PM Sunday–Thursday and 11:00 PM Friday–Saturday unless prior approval is given.
4.Residents are responsible for weekly chores as assigned by the house manager.
5.No overnight guests without prior written approval from house management.
6.All residents must be employed, actively seeking employment, or enrolled in a treatment/education program.
7.Residents must treat all housemates, staff, and visitors with respect at all times.
8.Destruction of property will result in financial liability and possible discharge.
04UA (Drug Testing) Policy

Residents are subject to random and scheduled urinalysis (UA) drug testing at any time. Testing may be conducted by house management or a designated third party.

A positive UA result, refusal to test, or tampering with a test will result in immediate discharge from the program.

Residents on prescribed medications must provide documentation from their prescribing physician within 48 hours of move-in.

05Visitor Policy

Visitors are permitted in common areas only during approved hours (10:00 AM – 8:00 PM). No visitors are permitted in resident bedrooms.

Residents are responsible for the behavior of their guests. Any visitor who violates house rules may be permanently banned from the property.

Overnight guests require prior written approval from house management and are subject to a background check.

06Property & Damage Agreement

Residents are financially responsible for any damage to the property, furnishings, or equipment beyond normal wear and tear.

Damage costs will be deducted from the security deposit. If damage exceeds the deposit, the resident is responsible for the remaining balance.

08Media / Image Release

RecoverPurpose may photograph or record activities at the house for organizational purposes. Your participation in any media is always voluntary and you may opt out at any time.

09Identity Verification
Government-Issued ID Photo

Accepted: JPEG, PNG, WebP, HEIC — max 10 MB. Upload a photo of your driver's license, state ID, or passport.

10Lease Agreement & Signature

I agree to abide by all house rules and policies as stated above.

I understand that violation of any rule may result in immediate discharge without refund of housing fees.

I agree to pay my housing fee on or before the 1st of each month.

I consent to random drug testing at any time during my residency.

I understand that this agreement is binding and that I am responsible for my conduct and the conduct of my guests.

Typing your name constitutes your electronic signature.

Your information is kept private and is never sold or shared outside of RecoverPurpose. Sensitive data is transmitted securely and not stored on this site.
